What are Primary Care Networks?

Primary Care is often your first entry point into the healthcare system. A long-term relationship with a Primary Care Provider (Family Doctor or Nurse Practitioner) supports your ongoing health and well-being.

Primary Care Networks (PCNs) are being developed across the province to support improved access to enhanced team-based care. In a PCN, a clinical network of providers in a geographic area work together to support their patients. This clinical network includes Primary Care Providers, Allied Health Supports, Nurses as well as other services in the community. Patient Medical Homes, or clinics that are optimized to provide long-term support for patients, are the foundation upon which PCNs are being built.

What is Team-Based Care?

Within a team-based model of care, multiple health care providers from different professional backgrounds work together and with patients/clients, families, caregivers and communities to deliver comprehensive health services across care settings. Effective teamwork is a critical enabler of safe, high quality care and allows health care teams to work to their strengths to provide the best care for their patients.

The Vancouver Primary Care Networks are enabled by a partnership between the Vancouver Division of Family Practice, Vancouver Coastal Health the BC Ministry of Health and community service providers. Together, we are working to improve the primary care system in Vancouver.
