Why We Exist
Primary Care Networks (PCNs) are bringing the community together to support your health and well-being. Together, we are enhancing the health of the community by introducing new team-based services, fostering collaboration, strengthening connections and improving access to care in Vancouver.
Our Partnerships
In Vancouver, PCNs are being built in partnership by the Vancouver Division of Family Practice and Vancouver Coastal Health. We are leaning on opportunities to collaborate, align health care goals and improve the primary care system for all.
PCNs in the province are also supported by the BC Ministry of Health and the First Nations Health Authority.
