Your Primary Care Network Health Supports

Primary Care Networks are increasing access to enhanced primary care by helping to optimize Patient Medical Homes, providing team-based care through new Allied Health services, and offering more options for urgent care in the community.

Patient Medical Homes

Your Patient Medical Home (PMH), is the clinic where you go to receive ongoing primary care. You may have access to other PCN supports as part of your clinic team.

PCN Allied Health Services

The PCN has introduced new Allied Health services, also known as the Interprofessional Team (IPT). These services are free of charge to eligible patients.

Urgent Primary Care Centres (UPCCs)

UPCCs provide same-day care for urgent, non-life-threatening health concerns. There are currently 4 UPCCs in Vancouver with extended hours to support you.
