PCN Clinical Pharmacists

PCN Clinical Pharmacists work directly with Primary Care Providers, as well as other health care providers, to contribute their drug therapy expertise to optimize the on-going care of patients with more complex needs.

How Can a PCN Clinical Pharmacist Help Me?

PCN Clinical Pharmacists can support you with more complex issues such as:

  • Multiple medications owith multiple health concerns
  • Optimizing medication you are taking for chronic conditions
  • Support when patients are taking 5 or more medications (polypharmacy)
  • Medication allergies or adverse reactions
  • Needing to improve medication outcomes
  • Complexities from self-treatment and supplements
  • Patient needing support after recently leaving the hospital or changing care
  • Financial hardships or other issues preventing patients from taking medication

What to Expect

The PCN Clinical Pharmacist will work with you and your Primary Care Provider to support your medication issues/needs. They will meet with you one on one virtually or in-person. Your Patient Medical Home may even have a PCN Clinical Pharmacist working on-site on certain days/times. Check with your Primary Care Provider.

Cost

There is no charge for the PCN Allied Health services. Patients who can access similar free support through Extended Health Benefits or other programs should utilize those options for care.

Care Delivery

Sessions are primarily virtual, but in-person appointments are available at two locations in Vancouver

Patient Eligibility

Speak with your Primary Care Provider in Vancouver to see if they have access to the PCN Interprofessional Team supports and if the services are a right fit for you.

  • Patient is attached to a Vancouver based Primary Care Provider (Family Physician or Nurse Practitioner) who has access to refer to PCN Allied Health Services
  • Patient needs are appropriate for short term, mild to moderate scope of care
  • Patient is 19 years and older
  • Patient is not able to access this support through other program or means (such as extended health benefits)
