PCN Occupational Therapists

PCN Occupational Therapists focus on helping people participate in the daily activities they need or want to do (occupations). They work with patients to help find solutions and address challenges to participating meaningfully in their lives.

How Can a PCN Occupational Therapist Help Me?

PCN Occupational Therapists work with patients with mild to moderate functional concerns. Their goal is to  prevent, address and/or minimize the effects of injury, illness, disability and social and environmental circumstances that interfere with your ability to do things like take care of your health, enjoy leisure time, and participate in your community.

An OT can support you to:

  • Develop new skills and healthy routines (e.g. sleep strategies, lifestyle changes)
  • Develop strategies to manage symptoms such as insomnia, fatigue, pain, or anxiety which impact daily activities (cleaning, cooking, etc.)
  • Stay healthy and active while managing a chronic condition
  • Learn how to do things differently, use equipment and adaptive aids, and optimize your environment
  • Maintain or regain your independence (e.g. falls safety)

What to Expect

The PCN Occupational Therapist will work with you and your Primary Care Provider to identify the activities that you are having trouble doing. They will complete an assessment, work with you to set goals and create a plan to work towards them. If needed, they can also refer you to other PCN Allied Health Supports.

Number of Visits

PCN services offer short-term (up to 6-8 sessions), person centred supports with an Occupational Therapist.

Cost

There is no charge for the PCN Allied Health services. Patients who can access similar free support through Extended Health Benefits or other programs should utilize those options for care.

Care Delivery

Sessions are primarily virtual, but in-person appointments are available at two locations in Vancouver

Patient Eligibility

Speak with your Primary Care Provider in Vancouver to see if they have access to the PCN Interprofessional Team supports and if the services are a right fit for you.

  • Patient is attached to a Vancouver based Primary Care Provider (Family Physician or Nurse Practitioner) who has access to refer to PCN Allied Health Services
  • Patient needs are appropriate for short term, mild to moderate scope of care
  • Patient is 19 years and older
  • Patient is not able to access this support through other program or means (such as extended health benefits)
