PCN Registered Dietitians

PCN Registered Dietitians are registered professionals who work with individuals to deliver nutrition care that is catered to their specific needs. The goal is to improve health outcomes by addressing nutritional needs and eating habits.

How Can a PCN Registered Dietitian Help Me?

PCN Registered Dietitians provide nutrition care to individuals who require support with:

  • Management of chronic conditions such as Diabetes, Heart Disease, High Blood Pressure, Osteoporosis, Liver or Kidney Disease, Gastrointestinal Disorders
  • Malnutrition and unintentional weight loss
  • Nutrition care for mental health and substance use concerns
  • Healthy eating strategies
  • Trouble with swallowing (if seen by a Speech Language Pathologist)
  • Controlling blood sugar
  • Nutrient deficiencies/imbalances
  • Food allergies/intolerances
  • Nutrition management of gastro-intestinal disorders (Irritable Bowel Syndrome, Celiac Disease, Crohn’s Disease, etc.)
  • Healthy eating on a limited budget

What to Expect

In collaboration with you and your Primary Care Provider, the PCN Registered Dietitian provides nutrition counselling as well as education to support you. They will make an assessment, develop a care plan, set goals with you and provide other supports. If needed, they can refer you to other PCN Allied Health Supports.

Number of Visits

PCN services offer short-term (up to 3 sessions), person centred supports with a Registered Dietitian.

Cost

There is no charge for the PCN Allied Health services. Patients who can access similar free support through Extended Health Benefits or other programs should utilize those options for care.

Care Delivery

Sessions are primarily virtual, but in-person appointments are available at two locations in Vancouver

Patient Eligibility

Speak with your Primary Care Provider in Vancouver to see if they have access to the PCN Interprofessional Team supports and if the services are a right fit for you.

  • Patient is attached to a Vancouver based Primary Care Provider (Family Physician or Nurse Practitioner) who has access to refer to PCN Allied Health Services
  • Patient needs are appropriate for short term, mild to moderate scope of care
  • Patient is 19 years and older
  • Patient is not able to access this support through other program or means (such as extended health benefits)
