Your PCN Social Worker will work together with you and your Primary Care Provider to address challenges and concerns in a confidential setting. If needed, they can also refer you to other PCN Allied Health supports.

PCN Social Workers provide information, counselling, and referral services to help people understand and manage medical, psychological, social and practical concerns impacting their lives.
How Can a PCN Social Worker Support Me?
PCN Social Workers help you to navigate and coordinate resources and services. They can help you with mild to moderate issues including:
- Mental Health & Substance Use supports
- Decision-making concerns and Advance Care Planning
- Indigenous cultural support
- Disability supports
- Financial resources and benefits
- Legal support
- Care coordination
- Health issues, medical benefits and prescription needs
- Housing
- Immigrant, refugee and newcomer supports
- Accessing affordable transportation
- Palliative end-of-life support
What to Expect
Number of Visits
PCN services offer short-term (up to 8 sessions), person centred supports with a Social Worker.
Cost
There is no charge for the PCN Allied Health services. Patients who can access similar free support through Extended Health Benefits or other programs should utilize those options for care.
Care Delivery
Sessions are primarily virtual, but in-person appointments are available at two locations in Vancouver.
Patient Eligibility
Speak with your Primary Care Provider in Vancouver to see if they have access to the PCN Interprofessional Team supports and if the services are a right fit for you.
- Patient is attached to a Vancouver based Primary Care Provider (Family Physician or Nurse Practitioner) who has access to refer to PCN Allied Health Services
- Patient needs are appropriate for short term, mild to moderate scope of care
- Patient is 19 years and older
- Patient is not able to access this support through other program or means (such as extended health benefits)
