What is a PCN?
A Primary Care Network (PCN) is a clinical network of providers in a geographic area where patients receive expanded, culturally safe, comprehensive care and improved access to team-based primary care. PCN networks include Primary Care Providers, Allied Health Supports, Nurses as well as other services in the community. PCNs are enabled by a partnership between a local division of family practice, the regional health authority, local First Nations, and other community partners. There are 6 PCNs across Vancouver, each covering a different geographical area. You can learn more here on our website.

What is a Steering Committee?
A Steering Committee plays a key role in providing strategic direction and oversight for an organization or initiative. There are six Steering Committees in Vancouver, one per PCN, that will offer governance and strategic oversight to each PCN. Each Steering Committees is comprised of two to three Family Physicians, 1 Nurse Practitioner and 1 Patient Partner. For more information click here.
