Primary Care Networks (PCNs) are being developed in the province to strengthen healthcare and connect the system to better support patient needs. They are leaning into team-based care, where multiple health care providers work together to provide the different elements of care that patients need. They are also sharing information and making connections so that people can access needed services in the communities where they live. There are 6 PCNs in Vancouver, each with distinct features and health care needs.
PCNs are being developed collaboratively by the Vancouver Division of Family Practice and Vancouver Coastal Health. This partnership connects the different areas of the healthcare system, what we call primary care – your first entry point into accessing health care supports, and what we call the acute system, which are services delivered in hospital as well as specialized community services. The PCNs were launched in Vancouver in 2019, and since that time we have been working on engaging health care providers and the community to support the big change we are making that reflects the growing healthcare needs of Vancouver.
