Vancouver PCNs bring together Primary Care Providers (Family Doctors and Nurse Practitioners), Allied Health Supports, Nurses as well as other community services to support the health and well being of the community.

There are 6 geographically based PCNs in Vancouver, each containing different healthcare and community supports.

The First Nations and Aboriginal (FNA) PCN supports Indigenous primary care needs through clinics located across Greater Vancouver and the Sunshine Coast.

PCN 1 City Centre

PCN City Centre includes the neighborhood of the Downtown Core, the West End and Fairview Slopes

PCN 2 Centre North

Centre North includes the neighborhoods of Grandview Woodlands, the Downtown Eastside and Northeast False Creek

PCN 3 Northeast

Northeast includes the neighborhoods of Hastings-Sunrise, Renfrew-Collingwood and Cedar Cottage

PCN 4 Westside

Westside includes the neighborhoods of Shaughnessy-Kerrisdale, West Point Grey, Kitsilano and UBC

PCN 5 Midtown

Midtown includes the neighborhoods of South Cambie, Riley Park, Kensington and Mount Pleasant

PCN 6 South

South includes the neighborhoods of Oakridge-Marpole, Sunset, Victoria-Fraserview and Killarney
