Your Primary Care Provider (Family Doctor or Nurse Practitioner) is your first point of care when you have a health concern or a question. The following locations in PCN 1 can provide more urgent or specialized care.

Community Based Supports

There are many community resources that can support your health and well-being in PCN 1. Neighborhood Houses, Seniors Centres, Community Centres and Libraries offer different types of programs to support their surrounding neighborhoods.

Gordon Neighborhood House

Located at 1019 Broughton St., Gordon Neighborhood House provides programs for all ages and offers free counselling, food justice programs and seniors programming and more to support the health and well being of the community.

Vancouver Public Library

Libraries in your neighborhood offer different healthcare navigation and support programming. You can search their events listings for healthcare related topics.

Community Centres

Community Centres provide a variety of programs and resources that can support health and wellness such as information sessions and fitness classes. Find the Community Centres in your neighborhood at the link below.

West End Seniors Network

Located at 1447 Barclay St., the West End Seniors Network enhances the quality of life of older adults by providing social, recreational, educational and supportive programs and services in the community.

South Granville Seniors Centre

Located at1420 West 12th Ave., South Granville Seniors Centre offers a variety of programs and services to support the health and well-being of seniors aged 55 and up in the community.
